What is Nextstellis?

Nextstellis is a novel birth control pill and the only one to have E4, a natural estrogen. It is described as NEST. 

What does NEST stand for?

N: Native

E: Estrogen

S: Selective action on

This means E4 has a selective action on tissues that supports cycle control, contraceptive efficacy, and other estrogen benefits. 

Most females avoid birth control pills due to their side effects. To combat this, Nextstellis has a natural form of estrogen with fewer side effects.

How to use Nextstellis

What are some Nextstellis label uses?

Nextstellis is an oral contraceptive pill for females who want to prevent pregnancy.

However, the medicine may become less effective for females with a BMI (body mass index) of 30 or above. Besides, it is not known if Nextstellis is effective and safe in women with a BMI of 35 or more.

What are some Nextstellis off-label uses?

Nextstellis may also be used to regularize periods, treat acne, decrease blood loss during periods, and lower the risk of ovarian cysts.

It may be prescribed for other uses; ask your doctor or pharmacist for more information.

Direction For Use of Nextstellis

  • Nextstellis is an oral pill to be taken once a day, probably the same time every day with or without food. 

  • The recommended dose is one pill for 28 consecutive days. (Pink or active pill for 24 days followed by white or inactive pill for 4 days.

No current use of contraception (Day 1 start):

  • Take the first pink pill on the first day of your menses
  • Take the subsequent pink pills at the same time every day for a total of 24 days
  • Take a white pill for 4 days at the same time every day that pink pills were taken
  • The next 28-day pack should be taken on the same day of the week as the first pack was taken (that is the next day of taking the last pill)

If you are not taking on the first day of menses:

Use another non-hormonal birth control method for the first seven days to allow the medication to work and prevent pregnancy.

If you switch from other forms of hormonal contraceptives, consult your doctor on how you can switch. Also, in case of any doubts, consult your doctor or pharmacist.

If you start after delivery:

  • Do not start before a month after delivery as it may increase the risk of clot formation.
  • If your menses have returned, begin with the day 1 method.
  • If your menses have not returned, use non-hormonal birth control for the first seven days as a backup.

If you are starting after miscarriage or abortion:

≤14 weeks gestation

  • Within the first week of complete first-trimester miscarriage or abortion, use additional non-hormonal contraception for seven days.
  • After the first seven days, follow the day 1 method.

> 14 weeks but ≤ 20 weeks gestation

  • Do not start before a month after miscarriage or abortion as it may increase the risk of clot formation.
  • If your menses have returned, start with the day 1 method.
  • If your menses have not returned, use non-hormonal birth control for the first seven days as a backup.

What to do if you miss a dose of Nextstellis?

  • If you miss one pink pill, take it as soon as you remember and take the next pill at its regular time. (You might take two tablets in one day)
  • If you miss two pink pills (during the first two weeks), take them on the day you remember. Take two pills the next day. Later continue as usual until you finish all the pills. Use a backup birth control method if you have sex in the week, you missed the tablets.

If you forget three or more pink pills or missed two pink pills during the third week:

  • Take one missed pink pill as soon as possible and take another pink pill for the current day and discard other missed tablets. Finish the active pills and discard the white inactive pills. Start with a new pack the next day.
  • Use another non-hormonal birth control method until the pink pill is taken for seven days to allow the medication to work and prevent pregnancy.

In case you miss periods for two consecutive menstrual cycles, you should consult your doctor.

Ingredients And Benefits in Nextstellis

Nextstellis is a monophasic, combination oral contraceptive birth control pill. Active ingredients are progestin drospirenone (3.0 mg) and estrogen estetrol monohydrate (15 mg).

The hormones in this pill prevent pregnancy by:

  • Preventing ovulation (release of the egg) during the menstrual cycle.
  • Making your vaginal fluid thicker to prevent sperm from reaching the egg.
  • Altering the uterine lining, preventing attachment of the fertilized egg.

Warnings and Precautions of Nextstellis

Talk to your doctor before taking Nextstellis if:

  • You are taking any other medication, are pregnant, breastfeeding, or planning a pregnancy. 
  • You are allergic to Nextstellis or any of its ingredients or have any other allergies.
  • You are allergic to any other medication.
  • You are taking any other drugs, over-the-counter medicines, herbal products, or nutritional supplements.
  • You wear contact lenses

Storage and Disposal Of Nextstellis

  • It is available as prefilled syringes and pens.
  • It should be refrigerated at 2ºC to 8ºC
  • Do not freeze
  • Do not shake to avoid foaming
  • Keep the product in the original carton until you want to use it.

Contraindications when on Nextstellis

Inform your healthcare provider in case of an allergy to Nextstellis or any other medication or any of the ingredients in Nextstellis. Ask your pharmacist or check the medication guide for a list of the ingredients. 

Do not take or inform your doctor if you have or had following conditions;

  • Clots in extremities, eyes, or lungs
  • Heart problems or heart attack
  • Stroke
  • Irregular heartbeat
  • Clotting disorders
  • Diabetes
  • Hypertension
  • Kidney failure or disorders
  • Severe migraine
  • Unexplained vaginal bleeding
  • Liver problems
  • Depression
  • Breast cancer or other cancers sensitive to female hormones
  • Adrenal glands disorders
  • Smoke or 35 years of age or older

Interactions with Nextstellis

It is best to avoid using Nextstellis with the following drugs:

  • CYP inducers such as barbiturates, griseofulvin, and topiramate
  • CYP inhibitors such as voriconazole, ketoconazole, and itraconazole
  • Ritonavir, telaprevir, and etravirine
  • Potassium-sparing diuretics
  • Lamotrigine
  • Corticosteroids
  • Potassium supplementation

What are some Nextstellis Side Effects 

Like all the medications, Nextstellis also has side effects. It is possible that you may experience the following side effects:

Common side effects

  • Mood changes
  • Breast pain, discomfort, tenderness
  • Irregular vaginal changes 
  • Headache
  • Acne
  • Decreased sex drive
  • Weight gain
  • Pain during periods

Severe side effects

  • Leg pain that does not subsides
  • Sudden changes in vision
  • Blindness
  • Shortness of breath
  • Palpitations
  • Nausea or vomiting
  • Chest pain
  • A severe, sudden headache
  • Numbness or weakness in your legs
  • Trouble speaking

Get emergency help if you have allergic reactions. The following are the signs of an allergic reaction: hives; swelling on the face, tongue, lips, or throat; difficulty in breathing.
