What is Evrysdi?

Evrysdi is the first oral medication approved to treat spinal muscular atrophy (SMA), a hereditary disease affecting the nerve cells of the brain and spinal cord.

How to use Evrysdi?

What are some Evrysdi label uses?

Evrysdi or Risdiplam is an oral prescription medication indicated in treating spinal muscular atrophy (SMA). SMA usually affects around one in every 10,000 infants. 

SMA is a rare and often fatal genetic disease affecting muscle strength and movements in persons aged two months or older. SMA is caused by gene mutation, which results in SMN protein deficiency in the brain. 

It is a hereditary disease that destroys nerve cells in the brain and spinal cord that control skeletal muscle activity such as walking, talking, swallowing, breathing, thus resulting in muscle fatigue and atrophy (decrease in size). 

Evrysdi is the first oral medication approved to treat SMA. 

What are some Evrysdi off-label use?

There is no off-label use of the medicine so far.

Evrysdi may be prescribed for other uses; ask your doctor or pharmacist for more information.

Direction for Use and Dosage

  • Evrysdi is available as an oral solution with a maximum dose of 5 mg to be administered once daily. 
  • The medication is to be administered orally once daily after a meal. It is to be administered using the provided reusable oral syringe.
  • Take medicine at the same time daily. 
  • Evrysdi is to be reconstituted by your pharmacist before dispensing. 
  • If you miss your dose, it should be administered as soon as possible without waiting, if it is within 6 hours of the missed dose. If it is more than 6 hours of the missed dose, skip the dose, and normal dosing can be resumed on the following day at the same time.
  • During the dosing, if vomiting occurs or if the dose is not entirely swallowed, dosing should not be repeated to make up for the lost dose. The dosing can be resumed the next day at the same time. 
  • Do not stop taking the medication without consulting the doctor. Take medicines as prescribed by your doctor. Do not take less or more than prescribed.
  • Make sure you swallow the entire dose, consume a glass of water after consuming the medication. 
  • The medication should be consumed immediately after it is drawn into the syringe. If it is not consumed within 5 minutes, the drug should be discarded from the syringe, and a new dose should be used. 
  • The medication is available in powder form; however, it must not be used in powder form. You must ask your pharmacist to reconstitute it into solution form. 
  • The medication is to be consumed using a reusable oral syringe, which is provided with the drug. Wash the syringe after each use and allow it to air dry. If you lose or damage the syringe, contact your pharmacist immediately and ask for a new one. 
  • If the medication goes into your eyes or on your skin, rinse your eyes with water and wash your skin with soap. 
  • If the medication is being administered to the child being breastfed, give the medication after feeding. Do not mix the medicine with infant formula or milk. 
  • Also, in cases where a person cannot swallow the medication, a nasogastric or gastrostomy tube can be used for administration. The tube should be flushed with water after delivering Evrysdi. 

What dose of Evrysdi to take?

Your physician will decide the dose depending upon the age and weight of the baby. The dose may range from 0.2mg/kg to 5 mg. 

How to store Evrysdi?

Store the dry powder at 20°C to 25°C. Keep the powder in the original carton.

Store the reconstituted solution in the refrigerator in an amber-colored bottle and protect it from light. Do not freeze the solution. Throw the unused reconstituted solution if not used for 64 days.   

Keep the bottle tightly closed and in the upright position when not in use.

Ingredients and Benefits in Evrysdi

Active ingredient: Risdiplam

Risdiplam works by addressing the underlying cause of SMA, i.e., increases in the production of SMN protein in the brain. 

Risdiplam Side Effects 

Like all the medications, Evrysdi also has side effects. It is possible that you may experience the following side effects that do not require medical attention;

Risdiplam Common side effects:

  • Vomiting
  • Diarrhea 
  • Skin rash
  • Sores
  • Ulcers
  • Constipation
  • Canker sores

Risdiplam Less common side effects:

  • Joint pain
  • Difficulty in moving
  • Muscle stiffness
  • Muscle pain

The mentioned side effects may not go immediately. Your physician or pharmacist may be able to tell you about ways to reduce or prevent the side effects. Check with them if they persist longer or if these side effects start bothering you or your baby.

Some of the effects mentioned below may not appear for everyone, but they may need medical attention. If you or your baby experiences any of them, please get in touch with your physician immediately:

  • Cough
  • Chills
  • Body pain
  • Chest tightness
  • Fever
  • Ear congestion
  • Loss of voice
  • Headache
  • Unusual weakness
  • Breathing difficulty 
  • Runny nose 
  • Stuffy nose 
  • Sneezing
  • Sore throat 

This is not a complete list of side effects. If you experience severe side effects or symptoms that are persistent, please contact your doctor immediately.

Contraindications

Inform your healthcare provider in case of an allergy to Evrysdi or any other medication or any of the ingredients in Evrysdi. Ask your pharmacist or check the medication guide for a list of the ingredients.

Do not take or inform your doctor if you have or had following conditions;

  • Liver disease
  • An adult man planning to start a family as it may affect fertility

Besides, inform your doctor if you are pregnant or breastfeeding, or planning to be pregnant. You should use birth control to prevent pregnancy during your treatment with Evrysdi and at least one month after your last dose, as the drug may be harmful to your unborn baby. If you become pregnant while you are on treatment with Evrysdi, call your physician immediately since Evrysdi may harm your fetus.

Talk to your doctor if you have concerns about the risk.  

Precautions

Inform your health worker about other prescription and non-prescription medications that you are taking. Also, inform about vitamins, herbal and nutritional supplements that you are taking or planning to take. 

The medication is to be used in adults and children aged two months or above. You may need a negative pregnancy test (females) before you start treatment with Evrysdi. 

Interactions

It may interact with the following medicines:

  • Methylnaltrexone
  • Metformin

This may not be a comprehensive list, and other drugs may interact with Evrysdi.

Is There A Generic Version Evrysdi?

No. For now, there is no generic version of Evrysdi available in Canada.
